By entering this contest, you (the entrant) agree to allow Lifetime Eyecare to publish your name and city and use your photo image on the LifetimeEyecare.ca website, Facebook and/or future Lifetime Eyecare E-Newsletters. The $500 Shopping Spree allows the patient or customer to use the $500 AS CASH towards any product or service at Lifetime Eyecare, 614 6th Street, New Westminster. However if the customer / patient chooses to return the product the $500 amount WILL NOT be returned as cash, but WILL BE allowed to be kept on file as CREDIT within the store. Only one award can be given to any particular entrant. All contest ballots are verified directly before a winner is announced.
